1)Basic:Annual contribution made based on years of service and gross pay
2)Matching:The first 3% of gross pay is matched by LGMC at the rate of $0.67 on the dollar every payroll.

Shift Differentials

Who Receives: All qualifying employees
Who Pays: LGMC
When Eligible: Immediately
What You Receive: From $0.60 to $5 per hour for alternate shifts

Merit Increase of Bonus

Who Receives: All eligible employees
Who Pays: LGMC
When Eligible: Annually
What You Receive: Annual evaluation and annual raise or bonus if meeting job criteria

Clinical Ladder

Who Receives: Eligible RNs
Who Pays: LGMC
When Eligible: 6 months employed
What You Receive: Bonus

Flextime

Who Receives: Full-time/qualified part-time employees
Who Pays: LGMC
When Eligible: After 3 months
What You Receive: Full-time employees receive up to 11 days after 1 year (credited service), up to 16 days after 5 years, up to 21 days after 10 years

Extended Illness Bank

Who Receives: Full-time/qualified part-time employees
Who Pays: LGMC
When Eligible: After 3 months
What You Receive: Full-time employees receive up to 6 paid days per year to be used after 24 consecutive hours of Flextime and/or "O" time for an extended illness, unless qualified for immediate access

Holidays

Who Receives: Full-time/qualified part-time employees
Who Pays: LGMC
When Eligible: Immediately
What You Receive: 8 paid holidays; 1 1/2 time paid for holidays worked in addition to 8 hours of holiday pay. Part-time receives 4 hours holiday pay if worked during pay period

Flexible Spending Accounts

Who Receives: Full-time/qualified part-time employees
Who Pays: You
When Eligible: After 60 days for full-time and/or 90 days for part-time
What You Receive: Pre-tax deductions for nonreimbursible health care and
dependent care expenses up to $5,000 annually
